This is our yearly Health / Permission release form that we collect for all participants for our own use. Whenever we go off campus or do a HFUMC only retreat we use these for the whole year so that we do not have to collect individual sheets all year long. It helps us & we hope it helps you. Please download fill out and return to the youth office as soon as you can.
